I understand exercise involves physical activity and some risk. I confirm the participant is fit to take part or appropriate medical advice has been taken. I confirm the participant will follow class instructions and stop if they feel unwell or unsafe. I understand Deb's Fitness may contact me about this waiver or my booking.

Photo and video permission

Deb's Fitness may occasionally take photos or videos during classes for use on the website, social media, and promotional materials. Your choice is optional and will not affect the participant's ability to attend classes.

I give permission for photos/videos of the participant to be used on:

Deb's Fitness website Deb's Fitness social media Printed or promotional materials

Permission can be withdrawn for future use by contacting Deb's Fitness.
